A quick chat with an electronics physicist has left me under the
impression that given the conductivity of copper, the diameter of the
conductors which run the local loop, a 50vAC driving voltage, and the
lower layers of the networking model, 45Mbit/s "over standard phone
lines" is just wishful thinking. Not even 4.5Mb/s. Right now a standard
modem has enough trouble pushing 56Kb/s down these cables. 

I've managed to get about 1Mb/s over phone grade copper on a private
circuit but I'm not clear how to make that 45 times faster with no
change in the parameters. What I have missed?
