Yep, only the centre two wires are used.  They are run straight through,
though sometimes companies request that their wall boxes be crossed to
stop people plugging phones in.  You can use the original (UK) cable
with a BT->RJ11 converter that has an internal capacitor.  Alternatively
you could just rig it up with the proper wiring.
