if I recall correctly BSD 4.3 didn't have runlevels, it just had /etc/rc
and /etc/rc.local; it must have been added since, or in 4.4 (good on yis
BSD for copying a good idea).

SysV had it since SVR4 at least, it's almost reasonable to say it was
their one good idea. ;)
