[ILUG] openssh problem

Hello again, I mailed the list a few days about an
openssh problem, since then i still have not had any
luck with it. I will include the log's errror messages
this time incase somebody see's what i am doing wrong.
I have created a key using ssh-keygen and then copied
this to authorized_keys, i did ssh-keygen again on the
other computer that i want to ssh from, then i
appended its identity.pub to the authorized_keys file
on the computer i want to ssh to.
I dont know if that the fact that i am ssh'ing from a
machine with ssh2 on it and the machine i want to ssh
to is using openssh, this should not be causing the
problem ?
Here are the error logs:
1. when i try to ssh remotely into the computer i get:
sshd[661] Failed password for username from ipaddress
port 714
2. when i try to ssh from the same machine to itself i
get:
sshd[520] Accepted rsa for username from ipaddress
sshd[520]Could not reverse map address ipaddress
sshd[520]PAM rejected by account configuration User
account has expired
sshd[520]Faking authloop for illegal user username
from ipaddress port 957
I have setup openssh before and did not have to do
anything special other than what i have done already,
this is why i am confused.
I also have a question, how are you allowed to ssh to
a box from say a ssh client on a windows machine when
the windows machine's identity.pub is not appended to
the hosts authorized_keys file ?
