Yes.

In KDE, open a KFM browser window. Edit -> mime
types. Select whichever folder best describe the
file type, probably "text". Right-click -> new
->Mime Type (or copy an existing one if it makes
you feel better). Fill in everything here.
Everything should be self explanatory except
"Default Application". Leave that for the moment.

Next Edit -> Applications. Pick a folder (if you
want). Now right click -> new -> application.
I've set mine up to run vi in a konsole, cos I
don't like gvim.

So in the execute box, I have "konsole -e vi".
Don't know if you'll need that structure or not.
You can specifiy mime types that your app can
open, but that's not too important. I think
that's for right-clicking files and getting a
list of possible viewers.

Chances are that the new MIME type hasn't been
spotted yet. If so, restart KDE. Go back to Edit
-> MIME types and now you can fill in the default
application for your new type as the new
application should now have been registered.

That's what I did. If anyone knows a better way
(and that doesn't include using emacs in the
example instead of vi), feel free to follow up.

Dale
