[ILUG] Sendmail: Can't create output and Insufficient Permissions

I'm getting this error in /var/log/maillog for a few
incoming mails to local users. 
Stats:  Sendmail 8.11
        kernel 2.4.2
maillog: Sep 13 21:17:36 leviathan sendmail[5388]:
f8DKHYc05387: to=<user at irelands-web.ie>,
ctladdr=<lingua-mortua at irelands-web.ie> (640/5000),
delay=00:00:02, xdelay=00:00:01, mailer=local,
pri=30273, dsn=5.0.0, stat=Can't create output


permissions: /var/spool/mail: drwxrwxr-x    2 root    
mail         4096 Sep 13 19:31 mail
                  /var/spool/mqueue: drwxrwxrwx    2
root     mail        77824 Sep 13 21:30 mqueue

Also getting from maillog:
