Update your kernel. There were failry critical updates to dm 
snapshots in 2.6.7 or thereabouts.

NB: you might need to edit your vg config, remove the snapshot 
definition and vgcfgrestore to remove the snapshot. Be sure to remove 
the snapshot before rebooting at least..
