a) Digests don't have keys. Though, digests often are used in
     conjunction with keys, in such modes these digests may still be
     useful.

b) taking file length into account might work if the message being
     'attacked' had no way to allow insertion of 'do nothing' padding
     (many message formats have some way to add padding) to allow the
     size of one to be made match the other.
