I'm using wvdial with an 6120 classic on O2 as I type.  The only
significant difference I can see with your wvdial config is that mine
includes:

 Init1 = AT+CGDCONT=1,"IP","internet"

It looks from your output that you're successfully connecting to the
access device on the other side.  I suspect the problem may lie with
your PPP configuration.  Check your /etc/ppp/options file.  Mine is:

 lock
 crtscts
 defaultroute
 usepeerdns
 ipcp-accept-remote
 ipcp-accept-local
 noipdefault

Adding 'debug' should increase the amount of detail fed to syslog.
