Did the reboot properly "mdadm --stop" the MD volume that failed the 
disk when the box came back?

dmesg and/or /var/log/messages?

If you've gone through and feel pretty sure the good disk [1] is 
actually good, just reconfigure it to be the new spare.

[1] There are replies before mine with good suggestions: check if any 
sectors were reallocated since release from factory; do some more work 
on it and see if the reallocation count increases afterwards.
